SomaSyncAI is not a medical service, medical device, or healthcare provider. The artificial intelligence features on this platform are designed exclusively to assist licensed practitioners with clinical documentation. They do not constitute medical advice, clinical diagnosis, therapeutic intervention, or any form of licensed healthcare service.
All AI-generated content must be reviewed and verified by a qualified licensed professional before use in any official patient record, billing submission, or clinical decision.
🏛 California AI Transparency Notice
In compliance with California's evolving AI transparency requirements, SomaSyncAI discloses that this platform uses artificial intelligence to generate clinical documentation. AI outputs are automated and may contain errors. Human review is required before use. We disclose our use of third-party AI infrastructure and the limitations of our AI systems in full below.
01

Critical Disclaimer
All content generated by SomaSyncAI — including SOAP notes, treatment summaries, session observations, and clinical suggestions — is for documentation assistance purposes only. It does not constitute medical advice, clinical diagnosis, or therapeutic recommendations of any kind.
SomaSyncAI does not establish a patient-provider relationship. No professional duty of care arises from the use of this platform. All medical and clinical decisions remain the sole responsibility of the licensed practitioner.
Before acting on any AI-generated content, practitioners should apply their own independent professional judgment and, where appropriate, consult current clinical literature and guidelines.
03
AI Limitations & Accuracy AI Transparency
SomaSyncAI discloses the following known limitations of our AI systems in the interest of transparency:
Hallucination: AI models can generate plausible-sounding but factually incorrect information. This is an inherent limitation of current large language model technology. All AI outputs must be critically reviewed.
Incomplete Context: Our AI only processes the information you provide. It has no access to a patient's complete medical history, diagnostic records, or external clinical data.
Training Data Limitations: AI outputs reflect patterns in training data, which may not represent the most current clinical guidelines or emerging research.
Bias: AI systems may reflect biases present in their training data. We actively work to identify and reduce these, but cannot guarantee bias-free outputs.
Speciality Limitations: Our AI is designed for general clinical documentation support. It may be less accurate in highly specialised clinical contexts.
Language & Dialect: AI voice processing accuracy may vary based on accent, speech patterns, background noise, or clinical terminology usage.
We are committed to ongoing improvement of our AI systems. If you encounter an inaccurate, inappropriate, or potentially harmful AI output, please report it immediately to feedback@somasyncai.com.
This is one of the most important obligations for SomaSyncAI users:
Mandatory Review Requirement
Every piece of AI-generated documentation — including SOAP notes, treatment summaries, and session records — must be reviewed and verified by the licensed practitioner before it is used in any official patient record, insurance submission, billing document, or clinical report.
Submitting unreviewed AI-generated documentation to patient records or billing systems may violate professional licensing requirements, HIPAA, and applicable California healthcare regulations. The practitioner bears sole responsibility for the accuracy and appropriateness of all documentation submitted under their name or license.
05
Voice & Audio AI California Disclosure
SomaSyncAI's primary AI feature involves real-time voice input processing. We disclose the following about how this works:
Voice input is processed in real-time by AI systems to generate structured documentation
Audio is converted to text; raw audio recordings are not retained beyond what is required for real-time processing
AI transcription accuracy depends on audio quality, speech clarity, and clinical terminology used
Voice-derived text data may be used in de-identified form to improve transcription accuracy
Patient Consent Reminder: California law and professional ethics require practitioners to obtain informed consent from patients before recording, documenting, or otherwise processing patient information during clinical sessions. The use of SomaSyncAI during a patient session is the practitioner's responsibility to disclose to the patient as appropriate.
06
Mental Health Boundaries
SomaSyncAI may be used by mental health practitioners for documentation support. Important boundaries apply:
Our AI does not perform psychological assessments, screening, or clinical evaluations
AI-generated session notes do not constitute a clinical formulation or treatment plan
The platform is not designed for crisis intervention and should not be used as a crisis resource
Crisis Situations
If a patient is experiencing a mental health crisis, suicidal ideation, or is in immediate danger, follow your professional emergency protocols and contact emergency services. SomaSyncAI is not equipped to provide crisis support.
Mental health practitioners using SomaSyncAI remain fully responsible for clinical judgment, risk assessment, safety planning, and all therapeutic decisions.
07

SomaSyncAI's features are powered in part by third-party AI infrastructure and large language model providers. We disclose this in accordance with California AI transparency principles.
Third-party AI providers process voice and text inputs to generate documentation outputs
We do not share identifiable patient data with third-party AI providers
All third-party AI partners are contractually bound to our data handling and confidentiality standards
We conduct ongoing review of third-party AI systems for safety, accuracy, and compliance
We do not endorse the independent outputs of any third-party AI system, and our use of third-party AI does not expand the accuracy or warranty guarantees described in this disclaimer.
